A Home Care Package is a Commonwealth Government funded program that gives you access to support services to assist you with staying at home with the support you need to ensure that you are able to do so safely and as independently as possible.

 

The Home Care Package program is consumer directed, which means we work with you to ensure you get what you need to meet what is important to you. It is your life, your choice, your way.

A Home Care Package offers:

• A wide range of services from personal care to health services, and even the purchase of consumables and equipment. 

• Access to a dedicated Care Advisor who will ensure you receive the most appropriate support possible, taking away the stress of managing your own care arrangements. ​

• Funds that are dedicated to your needs each year, putting you in control. If you don't use all your funds in the twelve month period, they will be rolled over into the next year for use. 

• Reassurance. Once you have a package, it stays in place for the long term. If your needs should change, we can help you apply for a higher level Home Care Package.

If you're over 65 or aged 50 years or older and identify as an Aboriginal or Torres Strait Islander person you are eligible to apply for an assessment for a Home Care Package. Our friendly staff can guide you through the process of applying for funding.
